before the Commission to present the Personal Property Add-On Report for August 2018.

Commissioner Williams moved to accept and enter into record the Personal Property Add-On
Report for August 2018 as presented. Commissioner Wyatt seconded the motion. The motion

N O W , T H E R E F O R E , in consideration o f the mutual prem ises and obligations set forth
herein, the C ou n ty and A gen t agree as follow s:

1. Term. The initial term o f this agreement shall b e for a period o f one (1 ) year from
O ctober 1, 20 1 8 , to Septem ber 30, 2019. The Coun';y, in its sole and absolute discretion, m a y
renew this agreem ent for tw o (2 ) additional o ne ) year tarn s b y providin g written n otice o f

2. Representations o f Agent. A gen t represents and warrants that it is a licensed
M issouri insurance producer.

3. S :o p e o f Services. A g en ; agrees to serve as a^eijt o f record on b e h a lf o f C ou n ty
in the evaluation, selection, and procurem ent o f p olicies c f hea'.tn insurance; providing usual and
custom ary services, inform ation and assistance w ith recpect to p olicies o f healtli insurance; as
w ell as those services identified in section 3 o r th s P.FQ.

4. Expiration or Term ination o f M em orialization. U pon expiraticn cr term ination o f
this agreement, or at any time upon request b y County, A geh t shall supply all inform ation
regarding C ou n ty insurance or County insureds’ , in-jli’.c ing crhployecs o f C o i nty, to any entity
designated b y County. A gen t understands thr.t failure to s a p -Iy inform ation a > requested b y
County w ill dam age C ounty and County em p loyees and ackn ow ledges its duty to prom ptly
supply all inform ation requested.

5. N o Financial C.bligation: C om oensat: on. N oih in g contained in this agreem ent is
intended, or should b e construed, as creating any financial obligation o f or b y County to A gen t, it
being understood that any com pensation for services b y A gen t is to b e in the form o f
com m issions to t e paid b y third parties and net County or C ounty em ployees. Further, the
appointment o f A g en t shall not impair the right o f any other agent o f record for County w ith
respect to com m ission s or com pensation earned b y , or due to, other agenis o f record.
